Notes System Overview
Clinical notes are linked to a client, classified under 9 clinical subcategories (Assessment, Formulation, Session, Planning, Crisis, Monitoring, Intervention, Documentation, Termination), and appear chronologically in the client flow.
Personal notes are not linked to any client, organized with folders (up to 3 levels deep), and have their own note types.
Both note types use the same editor, templates, and connection features.
Core note features include:
- Clinical blocks designed for therapists (therapist note, client quote, source quote, scale block, and more)
- Standard editor tools (headings, lists, tables, dividers, bold/italic/underline, and more)
- 100+ ready-made templates (SOAP, DAP, GIRP, BIRP and specialty-specific templates)
- Connections between notes and to goals
- Tagging
- Version history (up to 50 versions per note)
- Auto-save
- Import (.md, .txt; max 5 MB) and export (PDF, Markdown, Plain Text)
- Reflection notes for personal thoughts about the therapy process
Creating Notes
There are six entry points for creating notes:
- The create button at the bottom-right of the screen (accessible from any page)
- The quick action menu on the client detail page
- Two methods from the client flow:
- The button at the top of the flow for a client-linked note
- A session card in the flow for a session-linked note
- Directly from the session detail page for a session-linked note
- In another clinical note, select text and create a reflection note: the selected text moves into the new reflection note and the two notes are auto-linked
- From a goal's detail page, create a linked note (relationship type is chosen first)
For personal notes, the wizard follows type and template steps; for clinical notes, it follows client, type, and template steps. In both flows, you can quick-start with the standard note type without a template.
Clinical note wizard (3 steps):
- Client selection

- Template selection (choose a matching template or start with a blank template)

In both flows, a quick-start option is available with the standard note type; this skips the template selection step and opens the editor directly.
There are two ways to link a note to a session:
- From the note editor: open the Note Settings menu and switch to the Context tab; the client's sessions are listed and you can select one to link.
- From the client flow: open the menu on a note card and link the note to a session.
Once linked, the note appears on the session detail page and on the session card.
Notes Page
The Notes page consists of a left sidebar and a main content area. The Clinical and Personal tabs at the top switch between contexts.
Clinical context: the left sidebar shows a client list with sessions under each client; click a client or session to filter notes.
Personal context: the left sidebar displays the folder tree; click a folder to filter notes.
In the main area, notes are displayed as cards in a grid layout. Each context has three status tabs: Active, Archived, and Trash.
Filtering from the left sidebar varies by context:
- Clinical context: filter by client and session
- Personal context: filter by folder
At the top of the main area, there is a search bar for searching by title, content, and tags, a Filters button for accessing additional filters, and a sort option (such as last modified). You can also change the view mode from the top bar.
Note Types and Categories
Clinical notes are classified under 9 subcategories:
- Assessment
- Formulation
- Session
- Planning
- Crisis
- Monitoring
- Intervention
- Documentation
- Termination
There are 40+ system note types in total, and each subcategory has its own note types and templates. When creating a note, you first select the subcategory and then the note type within that category in the second step of the wizard.
Note Templates
Templates are formats that give notes a ready-made structure.
Each template uses a two-level hierarchy: blocks sit inside sections. Templates support 10 different block types.
Template selection happens in the third step of the note creation wizard; you can also apply a template to an existing note afterward.
Folder and Version Management
Archiving and Deletion
Notes can be in three states:
- Active: editable, visible in lists, all features available.
- Archived: preserved, read-only mode, can be reactivated when needed.
- Trash: deleted, can be restored within 30 days, permanently deleted after the retention period.
Reflection Notes
You can create one in two ways:
- Select text in any note and click the compass icon in the toolbar; the selected text is added as context and a reflection note is created.
- Create a new reflection note directly from the Notes page by selecting the reflection note type.
The first method is ideal for generating thoughts based on a specific clinical observation.
