What are Note Types?

Note types are the foundation of organizing and structuring your clinical documentation. Each note type is designed for a specific clinical need and contains templates you can use.

Category Structure

Mindora has two main categories: Clinical and Personal. Clinical notes are linked to client files, while personal notes are private to you.

Tip
Clinical note types are used in client notes, while Personal note types are used in your private notes only.

Clinical notes are divided into 9 subcategories representing different stages of the therapy process:

Assessment

Used for initial interviews, history taking, symptom analysis, and psychometric measurements.

Each note type contains different templates. For example, the Session Note type includes templates like SOAP, DAP, BIRP, GIRP, and MSE. You can see how many templates each type has in the list view.

Personal Notes

Personal notes are not linked to client files and are private to you. Use them to track your professional development, supervisions, and personal reflections.

Typical Workflow

The typical order of using note types in a therapy process:

System vs Custom Types

Mindora provides ready-made system note types. You can also create your own custom note types.

System Types

Evidence-based note types provided by Mindora that follow clinical standards. Cannot be edited but can be disabled.

Custom Types

Note types you create, customized to your needs. You have full control.

Default Types

Some note types are marked as "default". These types are automatically selected when you create a blank note.

Star icon

Shown with a star icon in the list view.

Restrictions

Default types cannot be edited, archived, or deleted.

Tip
Each category (Clinical and Personal) has a default type. This type is automatically selected when starting a new note.

Selecting Type in Note Editor

When writing a note, you can select the note type and template from the Settings panel.

Open the Settings panel on the right side of the note.

Switch to the "Type" tab.

Select the note type from the Strategic Type dropdown.

Select the desired template from the Template dropdown.

Tip
After selecting a template, click the "Apply" button. You can append the content to the existing note or replace the existing content.

Creating Notes from Goals

You can create notes linked to client goals. These notes are automatically associated with the relevant goal.

Go to the Goals section in the client profile.

Open the detail page of the relevant goal.

Click the "Create Linked Note" button.

Create the note by selecting a note type and template.

Tip
When creating notes from goals, only Clinical note types are visible. Personal note types cannot be linked to client goals.
