What are Note Templates?
Note templates are pre-built structures within each note type. A template consists of sections and blocks within those sections. Each block has a specific component type (text, checklist, table, etc.).
Mindora includes 90+ ready-to-use templates.
How Templates Work
When you select a note type while writing a note, templates for that type are listed. When you select a template, predefined sections and fields are automatically loaded into the editor.

Template Structure
Each template has a hierarchical structure: Sections and Blocks. This structure keeps your notes organized and consistent.
Sections
Sections are the main building blocks of a template. Each section has a title and contains multiple blocks. For example, a SOAP note has 4 sections: Subjective, Objective, Assessment, Plan.
Blocks
Blocks are data entry areas within sections. Each block has a title, optional description (instructional text), and a component type.
Each block can have gray instructional text below it. This text guides the therapist while writing the note and explains what to write in that field. You can also add instructional text to blocks in your own custom templates.
Component Types
There are 10 different component types used in templates:
Used for free text input. The most basic component of templates.

Contains checkable items. Multiple selections allowed.

Structured data entry with rows and columns.
Kullanım: Intervention planning, comparative analysis
Rating scale from 0-10 or 1-7.
Kullanım: Severity scoring, progress evaluation, SUD/VOC measurements
Special area with blue background and therapist icon. For clinical comments.

Area with golden background and quote icon. Italic text.
Kullanım: Client's important words, insights, verbatim expressions
Informational, non-editable content. For guidance purposes.

Special scale for behavior frequency (never → always).
Kullanım: Behavior tracking, habit monitoring
5 or 7-point rating for attitude and satisfaction measurement (Strongly disagree → Strongly agree).

Symmetrical scale from negative to positive (-5 to +5 or -3 to +3).

The most widely used clinical note format. Accepted as standard in medical settings.
Simplified version of SOAP. Combines subjective and objective data under "Data". Ideal for quick note-taking.
Format designed for goal-oriented therapy. Highlights treatment goals and tracks progress toward goals each session.
Format designed for behavior-focused therapy. Emphasizes observed behaviors and applied interventions.

Applying Templates
When applying a template to a note, you can choose between two modes:
Append Mode
Preserves existing content and adds the template to the end of the note. Ideal for combining multiple templates.
Replace Mode
Completely deletes existing content and replaces it with the template. Use when you want to start fresh.
Template Preview
Use the preview feature to see a template's content before selecting it.
Click on a template in the template list.
The template's section and block structure appears on the right side.
Each block's title, description, and component type is shown in the preview.
System vs Custom Templates
There are two types of templates in Mindora:
System Templates
Ready-made templates provided by Mindora. Cannot be edited, archived, or deleted. Marked with a lock icon.
Custom Templates
Templates you create. Fully customizable, can be edited, archived, and deleted.
Default Templates
Each note type has a default template. When creating a new note, this template is automatically loaded. For example, the default for Session Note type is SOAP Note.
Every note type has a "Blank Template" option. This template contains no predefined fields and is used for completely free-form writing. This way, you can categorize your notes by strategic types without being forced to use templates.
