Learn about text formatting, tables, semantic blocks, clinical scales, and display preferences in the note editor.
What is the Rich Text Editor?
Mindora's clinical note editor goes beyond plain text. It offers rich formatting options, clinical documentation blocks, and in-session measurement tools.
You can access everything from the toolbar or the '+' block insertion menu in the editor.
All content types in this guide (formatting, tables, semantic blocks, scales) are also available in the custom template editor. Create your own templates to have your frequently used structures ready to go.

Bold
Makes text bold.
Italic
Makes text italic.
Underline
Underlines text.
Strikethrough
Strikes through text.
Inline Code
Displays text in code format.
Editor Operations
When content is cleared, inline connections (relationships with notes and goals) within the note are permanently deleted.

Headings
H1, H2, and H3 heading levels can be selected from the Format Block dropdown. Use them to organize your notes into sections.
Alignment
You can align text to the left, center, or right.
Highlight Colors
You can highlight important sections with colors. We assigned a clinical meaning to each color for systematic documentation:
Yellow
General emphasis
Green
Positive, engagement
Blue
Information, neutral
Red
Attention, obstacle
Purple
Insight, creativity
Remove Highlight — Clears the highlight color from selected text.

When you select text, the selection toolbar appears with formatting (bold, italic, underline), link creation, reflection note creation, breakthrough marking, and semantic block conversion (client quote, therapist note) buttons.
Tables
You can create tables in the editor to present data in an organized way.
Insert Table
You can insert a table with a default 3×3 size from the '+' menu or toolbar.
Add/Remove Rows and Columns
You can add new rows or columns, and remove existing ones.
Cell Coloring
You can apply 6 different colors to cells: gray, yellow, green, blue, red, and purple.
Right-Click Context Menu
Right-click on a cell to access all table operations.
Cell Copy/Paste
You can copy and paste cell contents.
Click the arrow button in the top-right corner of the cell or right-click to access table operations.
Semantic Blocks
Semantic blocks are special content blocks that visually structure your clinical documentation.
Used to record the client's own words. Displayed with amber color and a quote icon.
Used for clinical observations and assessments. Displayed with blue color and a lightbulb icon.
Used for literature and supervision references. Displayed with teal color and a quote icon. The source can be linked to a note with a "Go to Source" button for quick navigation.
To add semantic blocks, open the '+' menu and select the Semantic Blocks category. You can also select text and convert it to a client quote or therapist note from the selection toolbar.
Semantic blocks visually organize your notes and structure clinical documentation.
@ References
Type @ in the editor to quickly reference your other notes and client goals. References appear as clickable inline links within your text.
Trigger
Type '@' anywhere in the editor. The search dropdown opens automatically.
Search and Select
Search results are grouped by Goals and Notes. Navigate with arrow keys and press Enter to select.
Reference Appearance
The selected item is inserted as an underlined '@Label' pill in the note.
Click and Navigate
Click a reference to open a detail popover — see the goal/note status, category, and a direct navigation button. You can also delete the reference from the popover.
Context-Aware Search — In clinical notes, only the same client's goals and notes are searched. In personal notes, only personal notes are searched.
@ References are lightweight inline links — they do not carry semantic relationships (Related, Supports, Opposes, etc.) and do not appear in the knowledge graph. Use connections for rich semantic relationships.

Clinical Scales
Clinical scales are in-session measurement and assessment tools. They can be embedded directly within notes.
Numeric Scale (1-10)
Linear 1-10 rating. Left and right endpoint labels (e.g. "Lowest" / "Highest") are customizable.
Likert Scale
Agreement-based assessment. Default 5 options: Strongly Disagree → Strongly Agree. Option count and labels are fully customizable (minimum 2).
Frequency Scale
Frequency-based measurement. Default 5 options: Never → Always. Option count and labels are fully customizable (minimum 2).
Bipolar Scale
0-100 slider scale. Left and right endpoint labels (e.g. "Sad" ↔ "Happy") are customizable. Default starts at center (50).
Common Features
All scales support title editing, description text, managing steps and options, reset, and deletion.
To add a scale, open the '+' menu and select the Scales category.
Click on the scale title and options to customize them as you wish.
Display Preferences
You can customize the editor appearance according to your preferences.
Font Size
Choose between Small (13px) or Normal (15px).
Font Style
Choose from Standard (Inter), Soft (Nunito), or Handwriting (Kalam) fonts.
Text Width
Switch between Focused (narrow) or Wide (full width) modes.
Focus Mode
Can be toggled from the toolbar. Reduces distractions.
Word Counter
Shows the live word count in the bottom-right corner.
Saving and Status
Changes you make in the editor are saved automatically.
Auto Save
Changes are saved to the server after a 1-second debounce while typing.
Status Indicator
Track saving status: "Saving..." → "Saved" → "Edited: X ago".
Manual Version Save
You can save versions to permanently preserve important moments. See the version history guide for details.
Important Notes
Use the '+' menu to add blocks — basic blocks, semantic blocks, scales, and table options are all there.
The selection toolbar that appears when you select text contains formatting, link creation, reflection, and breakthrough buttons.
All changes are saved automatically, no manual saving required.
