How difficult session follow-up and therapeutic impasse detection reminders work, their session modes, and configuration options.

Overview

This guide covers two session mode-triggered reminder rules:

  • Difficult Session Follow-up
  • Therapeutic Impasse Detection

Both belong to the Therapeutic Core category and appear in the Important tab on the Home page.

Tip
These reminders trigger the moment you save session modes. You can set modes from three places: the session detail page, the session card in the client flow, or the context panel in a session-linked note editor. The system evaluates on every save, regardless of where you make the change.

Why Does It Matter?

Difficult sessions and therapeutic impasses are among the most valuable learning opportunities for a therapist’s professional growth. Yet in a busy practice, these experiences can easily go unprocessed. Postponing a reflection note can lead to unrecognized countertransference reactions and recurring patterns.

These reminders alert you when a challenging session occurs or consecutive resistant sessions are detected, ensuring you process your own experience and review the treatment course. They help you catch the right timing for supervision or case consultation.

What Are Session Modes?

Session modes are labels that describe the general character of a session. You can select one or more modes for each session. These two reminder rules trigger based on the modes you select.

Session modes fall into three categories:

Content & Emotion

Breakthrough, Emotional, Exploratory, Psychoeducation, Skill Building, Crisis

Therapeutic Alliance

Difficult Session Follow-up

After challenging or emotional sessions, reminds you to write a brief reflection note to process and reframe the experience.

How does it trigger?

When you set a session’s modes (from session detail, client flow, or note editor context panel), the system compares that session’s modes with the configured modes. If at least one match is found, a reminder is created.

The session modes below trigger this reminder by default; you can add or remove them in the rule settings.

How to configure

Navigate to the Smart Reminders page from the top menu.

Click the gear icon next to the "Difficult Session Follow-up" rule.

Choose which session modes should trigger this reminder.

Change the default text shown on the card however you like.

Quick actions on the reminder card

With a single tap, you can do the following:

Create Reflection Note

Opens a new reflection note for the related client.

Go to Session

Navigates to the triggering session detail page.

Therapeutic Impasse Detection

When difficult session modes repeat across consecutive sessions, creates a reminder to re-evaluate the therapeutic approach.

How does it trigger?

When you set a session’s modes, the system looks beyond just that session and analyzes the pattern across recent sessions. If the configured modes appear in a certain number of sessions—including the one you just set (the "current session")—a reminder is created.

Current session matches

The newly saved session’s modes are compared against configured modes.

Previous sessions are checked

The system counts how many of the last N sessions (trend window) had matching modes.

Reminder created if threshold is met

If the number of matching sessions meets the threshold (including the current session), the reminder triggers.

The session modes below trigger this reminder by default; you can add or remove them in the rule settings.

Settings

You can adjust these values to match your clinical approach:

Trend window

The number of sessions looked back, including the current one, in which repeating difficult session modes are sought.

Default: 5 sessions

Trigger threshold

The number of sessions within the trend window that must have matching modes for the impasse reminder to trigger.

Default: 3 sessions

Default reminder text

When the reminder triggers, the card shows the following text by default; you can edit it however you like.

"Recurring difficult session patterns detected across recent sessions. Consider re-evaluating the therapeutic approach."

How to configure

Tip
You can fine-tune sensitivity by adjusting the trend window and threshold. A larger window looks at a broader period; a lower threshold triggers earlier.

Tip
A therapeutic impasse can be an important signal for supervision or case consultation. Recurring difficulties may indicate it’s time to review the therapeutic relationship and treatment plan.

Difference Between the Two Rules

While the two rules may look similar, they serve different scenarios:

Frequently Asked Questions

What happens if I select multiple modes for a session?
If at least one of the selected modes matches the configured modes, the reminder triggers. All modes don’t need to match.
Can the same session trigger both Difficult Session and Therapeutic Impasse?
Yes. The two rules work independently. A session can trigger Difficult Session, and if the historical pattern also matches, Therapeutic Impasse can trigger simultaneously.
If I change the session modes later, does the reminder update?
When session modes are updated, the system re-evaluates. If the new modes don’t match, no reminder is created; if a match already exists, no duplicate is added.
What exactly does the trend window mean?
It’s the number of sessions to look back, including the current session. For example, a window of 5 means the current session + the last 4 sessions are checked.
What does the threshold (trend count) represent?
It’s the minimum number of sessions within the trend window that must have matching modes. The current session is included in this count.
Are cancelled or no-show sessions included in the trend calculation?
No. Cancelled and no-show sessions are automatically excluded from the trend calculation.
Can the Therapeutic Impasse reminder trigger multiple times for the same client?
Yes. There is no spam guard; a new reminder is created each time the pattern is met. If needed, you can pause the rule for that client.
Are reminders visible to clients?
No. Reminders are only visible on the therapist’s Home page.
Can I use a custom session mode as a trigger?
No. Session modes are system-defined and cannot be customized. However, you can choose any combination from the 14 available modes.
What happens if the trend window is set to 1?
Historical checking is disabled and the rule triggers based on the current session alone, similar to Difficult Session Follow-up.
How do I remove a false positive reminder?
Click the ⋯ menu on the reminder card and select "Completed". If false positives recur frequently, consider removing the triggering session mode from the rule configuration.
If the current session’s modes don’t match, does Therapeutic Impasse still check history?
No. The system checks the current session first. If its modes don’t match the configured modes, history is never checked and no reminder is created.
Can I pause these rules for a specific client?
Yes. Use the ⋯ menu on the reminder card and select "Pause for this client". That rule will no longer trigger for that client. You can re-enable it from the rules page.
If I save the same session’s modes multiple times, does it create a new reminder each time?
No. The system won’t create a new reminder if an active or snoozed one already exists for the same rule and client. However, if you mark the existing reminder as "Completed" and then save the modes again, a new reminder can be created.
If Therapeutic Impasse triggered and the next session also meets the threshold, does it trigger again?
If the previous reminder is still active, no — the system won’t create a duplicate for the same rule and client. But if you marked the previous reminder as "Completed" and the threshold is still met, a new reminder will be created.
Does changing modes on a very old session trigger Difficult Session?
No. Difficult Session Follow-up only creates reminders for sessions within the last 14 days. Mode changes on older sessions won’t trigger this rule.
