Crisis and Risk Reminders

How crisis plan, hopelessness, dissociation, and relapse detection reminders work, their default keywords, and configuration options.

Overview

This guide covers four critical note-triggered reminder rules:

  • Crisis Plan Reminder
  • Hopelessness Detection
  • Dissociation Detection
  • Relapse Detection

All four belong to the Therapeutic Core category and appear in the Important tab on the Home page.

Tip
These reminders trigger automatically as you write notes. When configured keywords appear in a session note, progress note, or any clinical note, a reminder is created.

Why Does It Matter?

Crisis moments are the most critical junctures in therapy. When a client expresses suicidal ideation, self-harm, or serious regression, a therapist’s swift and accurate response can be life-saving. Yet in a busy clinical practice, these signals can be missed — especially when notes are written at different times or caseloads are heavy.

Crisis and risk reminders step in precisely at this point: they automatically detect critical expressions in your notes and remind you of urgent next steps. This ensures vital interventions — creating a safety plan, exploring hopelessness in depth, or revising the treatment plan — are never delayed.

How It Works

All four rules share the same mechanism: when you save a note, the system compares its content against the configured keywords. If a match is found, a reminder is created for the relevant client.

You write a note

You write and save content in any clinical note type (session note, progress note, etc.).

System scans for keywords

The note content is compared against the rule’s keywords in a case-insensitive manner.

Reminder is created

If a match is found, a reminder card appears in the Important tab on your Home page.

Only one reminder is created per note per rule. Even if multiple keywords match in the same note, you’ll see a single reminder.

Crisis Plan Reminder

When critical keywords appear in your notes, creates a reminder to review or create a crisis and safety plan with the client.

By default, this rule scans your notes for the keywords below; you can add or remove them to fit your practice.

Tip
The crisis plan reminder helps you prioritize client safety. It triggers when suicidal ideation or self-harm language is detected, prompting your emergency response plan.

Create Crisis Note

Opens a new crisis plan note for the related client.

View Note

Opens the triggering note and highlights the matched keyword.

Hopelessness Detection

When hopelessness-related keywords appear in your notes, creates a reminder to gently explore this with the client.

Tip
Hopelessness is a significant indicator of depression and suicide risk. Early detection and therapeutic intervention are critically important.

Tip
Dissociative symptoms may point to a trauma history. Stabilizing the session with grounding techniques and conducting a safety assessment is important.

Regression can be a natural part of the treatment process. Catching it early and reviewing the treatment plan supports recovery.

Frequently Asked Questions

Is keyword matching case-sensitive?
No. Keyword scanning is case-insensitive. "Suicide", "suicide", and "SUICIDE" all match the same way.
What happens if multiple rules trigger on the same note?
Each rule works independently. If a note contains both "suicide" and "hopeless", two separate reminders (Crisis Plan + Hopelessness) are created.
What if multiple keywords of the same rule match in one note?
Only one reminder is created. The one-reminder-per-note-per-rule principle applies.
Does editing an old note trigger a new reminder?
When a note is updated, the system re-scans it. If no reminder was previously created for that note + rule combination, a new one may be generated.
Are reminders visible to clients?
No. Reminders are only visible on the therapist's Home page. Nothing is shown on the client portal or client side.
How do I remove a false positive reminder?
Click the ⋯ menu on the reminder card and select "Completed". You can also remove the keyword causing false positives from the rule configuration.
Can I disable these reminders for a specific client?
Yes. Click the ⋯ menu on the reminder card and select "Pause for this client". That rule will no longer trigger for that client.
If I change the reminder text, do existing reminders update?
No. Text changes only affect newly created reminders. Existing reminders keep the text from when they were created.
Can I add keywords in languages other than English?
Yes. You can add keywords in any language. Therapists who write notes in multiple languages can use keywords in both.
Can I completely disable these rules?
Yes. On the Tools → Smart Reminders page, use the toggle next to each rule to enable or disable it.
Can the crisis plan reminder send a push notification instead?
Currently, crisis reminders only appear in the Important tab on the Home page. Push notification or email integration is not yet available.
Can I restore a deleted keyword?
Yes. Open the rule configuration via the gear icon and re-add the keyword. Default keywords are always available as a reference in this guide.
If I delete the triggering keyword from the note, does the reminder disappear?
No, the reminder stays. When you navigate to the note from the reminder card, you’ll see a notification that the triggering keyword is no longer present. If the keyword is still in the note, it will be highlighted in orange.

Tips

Mind your note language

The system scans your notes. If you use clinical jargon, adjust keywords accordingly.

Reduce false positives

Overly generic words can generate too many reminders. Use more specific terms.

Prioritize the crisis plan

The crisis plan reminder is the most urgent. We recommend keeping this rule always enabled.

Default text is a guide. You can write text that matches your clinical style.
